Description

Work in conjunction with the Care Coordination Director and staff to coordinate care needs and close care gaps for the assigned patient population.

Preferred Qualifications:

  1. Four years working in ambulatory care settings.
  2. Experience working with population health.
  3. Global comprehension of medical care settings, such as acute, ambulatory, and post-acute settings.
  4. Certification from the American Association of Medical Assistants.

Required Knowledge, Skills, and Abilities:

  1. Graduate of a program for Medical Assistants or equivalent education.
  2. Two years experience working in a medical facility.
  3. Knowledge of medical terminology, common chronic diseases, and preventive health.
  4. Skill in medical record documentation.
  5. Ability to utilize independent and sound judgment to accomplish assigned responsibilities.
  6. Ability to self-direct and multitask.
  7. Ability to be flexible and adaptable to changing assignments and work pace.
  8. Ability to recognize problems and share ideas for solutions.
  9. Sound judgment and decision-making skills.
  10. Strong written, verbal, and telephone communication skills.
  11. Excellent organizational skills with the ability to pay close attention to detail.
  12. Ability to work in a high-volume, fast-paced environment.
  13. Ability to establish effective working relationships with staff members, providers, patients, and outside agencies.
  14. Ability to use a computer. Proficient keyboarding skills. Ability to type at least 30 words per minute. Microsoft Windows proficiency, including Word, Excel, and Outlook.

Pay Ranges

Base Hourly: $21 per hour
Annual: $44000 – $54000

Education & Experience
