Description
The behavioral health consultant is a core member of the collaborative care team, including the patient’s medical provider, mental health consultants, as well as the larger primary care team, and the Comprehensive Care Coordination Department. The behavioral health consultant is responsible for providing direct patient care, supporting and coordinating the mental and physical health care of referred patients with the patient’s medical provider and, when appropriate, mental health providers. Appointments are brief and targeted, requiring succinct documentation and brief functional assessment.
Typical Requirements:
- Master’s degree in Social Work. Licensed Clinical Social Worker.
- Demonstrated ability to collaborate and communicate effectively in a team setting.
- Maintaining effective and professional relationships with patients and other care team members.
- Experience with screening for common mental health and/or substance abuse disorders.
- Working knowledge of differential diagnosis of common mental health and/or substance abuse disorders.
- Experience with assessment and treatment planning for common mental health and/or substance use disorders.
- Ability to effectively engage patients in a therapeutic relationship.
- Ability to work with patients by telephone and video visit, as well as in person.
- Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation).
- Basic knowledge of psychopharmacology for common mental health disorders.
- Experience with evidence-based counseling techniques.
- Ability to utilize computer programs for patient care and access information from the internet.
Preferred Qualifications:
- Two years of professional LCSW experience, preferably in an ambulatory care setting.
Pay Ranges
Base Hourly:
$35 per hour
Annual:
$73000 – $95000
